<p><span style="font-weight: 400;">Vous accompagnez un proche en situation palliative &agrave; domicile.</span><span style="font-weight: 400;"><br /></span><span style="font-weight: 400;">Le m&eacute;decin de votre proche a &eacute;tabli avec lui un <strong>projet de soins &agrave; domicile</strong>, afin de d&eacute;terminer les actions &agrave; mener pour l'accompagner au mieux tout en respectant ses choix.</span></p><p><span style="font-weight: 400;">La maladie de votre proche &eacute;tant grave, il est possible que vous ayez besoin d'appeler une &eacute;quipe m&eacute;dicale en urgence ou le 15.</span></p><p><span style="font-weight: 400;">Le m&eacute;decin de votre proche malade vous a remis une <strong>enveloppe &agrave; remettre &agrave; tout m&eacute;decin intervenant en urgence au domicile</strong>.
